The Search for
Unsung Heroes
of the African American Community

Nomination Form

Do you know anyone who lives or works within the African American community, adult or teenager (ages 13–18), who unselfishly gives of his or her time and energy to make life better for others? That person may be a good candidate for one of the awards which the San Francisco Public Library will present on Sunday, Nov. 23, 2008. The Unsung Hero Awards will honor individuals from the African American community who may not receive much publicity, recognition or money for what they do. Now is our chance to thank them. Nominations for the awards must be received by November 3, 2008.

A committee of community representatives will determine the winners. Anyone but the committee may suggest candidates (children too!). If there is someone in your organization, church, school, family or neighborhood who deserves an award, please use this form to tell us about him or her. Come to the program on Sunday, November 23 at 1 p.m., in the Koret Auditorium on the lower level of the Main Library, located at 100 Larkin St. at Grove. For more information, please contact Linda Brooks-Burton at (415) 355-2851 or Stewart Shaw at (415) 557-4518.
