Heart Bridge Care is a San Francisco Bay Area nonprofit founded and directed by hospice nurses that provides compassionate end-of-life care navigation, holistic support, and community education. The organization helps individuals and families navigate the medical, emotional, logistical, and spiritual challenges of serious illness, aging, and the dying process, while training the next generation of end-of-life doulas and building greater community understanding of end-of-life care.
